Hospital Need a New "Use Case"

“Shift in care settings” is more important than ever: Value-based care is coming, and hospital buildings need a new use

“All of my investment now is in outpatient—anything that is not beds. Our ability to create that network outside the hospital is huge.” - CEO of a healthcare system

“The focus of the past 10 years has been on preventing readmissions, and now in the next 10 years, the focus will be on preventing admissions.” - President of a health system

“Our mission is to provide health care to the community. If we can deliver care to the community in a better-quality manner that is less capital intensive, then that is better for our fiduciary commitment to the community.” - Population health executive of a health system

Hybrid Trials in Progress Post Pandemic

Association of a Bundled Hospital-at-Home and 30-Day Postacute Transitional Care Program With Clinical Outcomes and Patient Experiences

Mount Sinai tested the model from 2014 to 2017 as part of a demonstration under the Center for Medicare and Medicaid Innovation.

The study included 295 hospital-at-home care patients and 212 control patients

Hospital-at-home patients had an 8.6% 30-day readmission rate, which was lower than the 15.6% readmission rate for the control group of acute-care hospital patients.

Hospital-at-home patients had a 5.8% 30-day emergency department revisit rate versus an 11.7% ED revisit rate among the control group

HaH care bundled with a 30-day post-acute transitional care episode was associated with better patient outcomes and ratings of care compared with inpatient hospitalization. This model warrants consideration for addition to Medicare's current portfolio of shared savings programs.
